we have Care Coordinators based in Narrogin, Merredin & Moora who can visit surrounding towns in the Wheatbelt region
our Care Coordinators are trained in chronic disease care and can help you to manage your condition
our Care Coordinators will work closely with your GP and any allied health providers you may use
a referral and management plan from your GP is needed
