ALBANY CONNECTED COMMUNITIES PROJECT (ACCP)
The ACCP supports those with unmet social needs who are:
over 18 years of age and/or,
living with a chronic illness and/or,
needing support with lifestyle changes.
The Amity Health Link Team helps people to enjoy life to the fullest by connecting them with the support they need.
Many older adults face challenges such as financial strain, social isolation, food insecurity, transportation issues, low health literacy, unstable or insecure housing and inadequate employment. By connecting program participants to the right support services, we aim to help them to lead a healthier and happier life.

COMMUNITIES FOR CHILDREN PROGRAM
Our Communities for Children program is funded by the Australian Government. This enables us to provide funding and support to our community partners in the Great Southern to deliver programs for children and families to support their health, wellbeing and community connections.

Our Community Enrichments Grants of up to $1000, are for community groups and not-for-profit organisations to deliver an activity to support children and families in your community.

KIDS HUB MERREDIN
The Amity Health Kids Hub Program is based at Merredin College, where we offer a nurturing environment for families with children aged 0-12 years. Our program is dedicated to supporting kindergarten readiness, fostering early years development, and creating memorable experiences through school holiday programs and family-centric community events.
We prioritize the health, activity, and connectivity of children and families within our community. Led by our dedicated Family Support Workers, our playgroup activities are designed to be both enjoyable and educational, promoting social and emotional wellbeing while nurturing children's development during these formative years.
At the heart of our program lies our commitment to building a stronger, more resilient community. Through principles of social inclusion, collaborative leadership, and active community engagement, we aim to create a supportive network that benefits everyone involved. Join us in shaping brighter futures for our children and our community.

CHILD & PARENT SUPPORT (CAPS) PROGRAM NARROGIN
The Narrogin CAPS program is designed specifically for families with children aged 0-12 years. Our mission is to enhance the wellbeing and development of children while fostering connections among parents and carers, and other community service and health providers.
For the little ones aged 0-4, we host engaging weekly playgroups coupled with valuable parenting support. Meanwhile, older children aged 4-12 can join our lively after-school hub club, providing them with a safe and enriching environment. Additionally, we organize various events and activities for the whole community to participate in and enjoy. Explore the opportunities to grow, learn, and connect with us in Narrogin.
SPEECH PATHOLOGY & OCCUPATIONAL THERAPY OUTREACH PROGRAM
With the support of Rural Health West, Amity Health provides paediatric allied health outreach services to the communities of Jerramungup and Walpole. Once per term our Speech Pathologist and Occupational Therapist visit the Jerramungup and Walpole Primary Schools to assess all children to identify where additional support may be needed.
