Integrated Chronic Disease Care Program (ICDC)

OFFERED IN: Bindoon, Gingin, Jurien Bay, Lancelin, Merredin, Moora, Narrogin, Southern Cross

REFERRAL: requires a referral from your GP or other allied health practitioners, and a GP Management Plan.

Connections Count - Mental Health Counselling in Residential Aged Care Facilities

AGED CARE

The care finder program

OFFERED IN: Chittering, Cunderdin, Dandaragan, Dowerin, Gingin, Merredin, Moora, Muckinbudin, Northam, Toodyay, Wagin, York-Beverley

REFERRAL: eligible people can self-refer to the Amity Health care finder program

ABORIGINAL HEALTH & WELLBEING

Integrated Team Care Program (ITC)

Support for clients with chronic disease to link them to the support they need to better manage their health

REFERRAL: A GP or health practitioner referral is required

Mental Health, Wellbeing & Resilience Program

This community engagement program is designed to support the physical and psychological health of Aboriginal communities in the Wheatbelt region.

PARENTS, CARERS & FAMILIES

Amity Health Kids Hub Merredin

For parents and/or carers of children aged 0-4 years, living in Merredin and surrounds. The Kids Hub provides a weekly playgroup to nurture the growth of children, and provide support and helpful parenting tips to parents and carers.

Child & Parent Support Group Narrogin

For families with children from 0 - 11 years old, providing weekly playgroups and parenting support to improve families wellbeing and child development.
