REFERRAL: requires a referral from your GP or other allied health practitioners, and a GP Management Plan.

Integrated Team Care Program (ITC)

Support for clients with chronic disease to link them to the support they need to better manage their health

Mental Health, Wellbeing & Resilience Program

This community engagement program is designed to support the physical and psychological health of Aboriginal communities in the Great Southern region.

OFFERED IN: Gnowangerup, Mount Barker, Tambellup

Strengthening Aboriginal Health Program

A partnership with general practice and Great Southern Aboriginal Health to provide clinics for clients to have their health assessed and checked regularly.

PARENTS, CARERS & FAMILIES

Communities for Children Program

Our Communities for Children program is funded by the Australian Government.  This enables us to provide funding and support to our community partners in the Great Southern to deliver programs for children and families to support their health, wellbeing and community connections.

Walpole Speech Pathology and Occupational Therapy Outreach Program

With the support of Rural Health West, Amity Health provides paediatric allied health outreach services to the community of Walpole. Once per term our Speech Pathologist and Occupational Therapist visit the Walpole Primary School to assess children experiencing difficulties with speech, language, and sensory-motor development.
